Post-Hospital Extended Care
Our goal is to help our residents in the recuperation process while helping them achieve their maximum capabilities to return home as quickly as possible. Our nurses, therapists, dietitians, and case managers are available 24 hours a day. Within our post-hospital extended care, we also offer social services as well as enrichment activities.

Hanford Post Acute employs outgoing and friendly professional staff to create social programs and activities to fit the residents’ needs.
