If you or a loved one is in need of a higher level of care or is at risk for readmission to the hospital, choose us! We can admit to our facility in less than 30 days if that patient has had a 3-night qualifying hospital stay. We work with many insurance companies and will do our best to get you here for your safety any way we can.
Upon admission, we will have paperwork that needs to be completed in a timely manner. Our admissions coordinator will reach out to you and your family to set up a time to complete this. Please set aside a good hour for the meeting.

- Insurance cards
- Medicare/Medicaid cards
- Power of Attorney Documents (if applicable)
- Form of payment (if applicable)
